What do I need to know?

We encourage all patients to check with their insurance if we are in-network or not. Most procedures we offer are covered and we will request authorization with your insurance for procedures. Please bring your medical and prescription drug insurance cards to your appointment every time.

What should you bring?

Be sure to have your current insurance card (bring all insurance cards including Medicaid card and prescription insurance card if applicable), a photo ID, and a current list of medications you are taking. You can create an account through our Patient Portal and access some of the necessary forms ahead of time.

How do I pay?

You may make payments online through our Patient Portal.
Copays and previous account balances are expected to be paid on the day of your appointment and payment plans are available if needed.

We accept all major insurance plans, however, we are out of network with CHI network plans.
